About The Service
The Early Years and Disability Inclusion Team work with, support and coordinate services for children with additional needs and disabilities, their families and settings/schools to ensure that children access their entitlement with smooth transitions into childcare/school. We work in partnership with parents, education services, and other professionals in education and health.
The Early Years and Disability Inclusion Team monitors and supports schools/settings to successfully include all children with additional needs and disabilities to access their entitlement, environment and make progress.
About the job
An exciting opportunity has arisen for a Transition Worker, working as part of the Disability and Early Years Inclusion Team within the Education Service and with partners in many different agencies. The role focuses on working with parents/carers ensuring that the needs of children with additional needs and disabilities are supported to access their entitlement and make good progress.

We are looking for a highly organised, motivated and passionate individual who can join our dynamic forward thinking team to improve outcomes for children through partnership working.
