We are looking for a committed and compassionate Family Support Team Worker to join our dedicated team in Pembrokeshire. This is an exciting opportunity to play a vital role in delivering early intervention, prevention, and support services to families with children and young people aged 0-25.

About the Role

As a Family Support Team Worker, you will act as a key worker for families, building trusted relationships and working in partnership to:
  • Identify strengths, needs, and aspirations
  • Develop tailored support plans
  • Strengthen informal and formal support networks
  • Improve outcomes for children and young people
  • Enable families to achieve their goals and reach their full potential
  • You will work directly with families across Pembrokeshire, providing practical guidance, early help, and preventative support to reduce escalation to more intensive services.
This role contributes to the wider preventative aims of the Children and Communities Grant, supporting sustainable, community-based solutions for families.

What You'll Be Doing
  • Delivering high-quality, strengths-based key worker support
  • Completing assessments and co-producing action plans with families
  • Working collaboratively with partner agencies and professionals
  • Supporting families to access appropriate services and community resources
  • Maintaining accurate records and contributing to service evaluation
  • Undertaking key worker responsibilities as agreed in supervision
What We're Looking For

We're seeking someone who:
  • Has experience working with children, young people, and families
  • Understands early intervention and preventative approaches
  • Can build positive, professional relationships based on trust
  • Is confident working independently and as part of a multi-agency team
  • Has strong communication, organisation, and problem-solving skills
  • Is committed to safeguarding and promoting wellbeing
A relevant qualification in childcare, health & social care, youth work, or a related field is desirable.
