The role:

Enable the College to be an active party in the transition process for learners with additional learning needs (ALN) through attending reviews and planning for a successful transition. Monitor the effectiveness of the support provided through conducting annual reviews using person-centred practice.

37 hours per week
Fixed-Term until 31st August 2024
£26,621 - £29,297 Per Annum

Key Responsibilities:

To liaise with the education and advice providers e.g. Careers Wales, local schools and care providers with the aim of identifying appropriate provision for young people in College.

To engage with potential new learners from local schools and other agencies and create meaningful transition opportunities for learners with Additional Learning Needs and Disabilities (ALND) into college, whilst proactively responding to the changing governmental strategies and policies for this area.
Hold IDP/LSP reviews using person centred techniques and ensure that all relevant stakeholders are identified and have an opportunity to contribute.

About you:

Proven experience of working with young people with ALN in group and individual settings in a post 16 environment.
Experience of multi-agency liaison and signposting.
Empathic and responsive to learners needs, being able to motivate, engage and enthuse learners in innovative ways.
